HEALTH TEAM-
Ø Health team is defined as a group of
persons who work in coordinated manner to achieve a common goal of helping a
person in health or illness to promote health, prevent illness and recovery
from the condition.
COMMUNITY HEALTH
TEAM-
Ø Community Health team is defined as a group
of persons who share common health goals determined by community needs and each
member work and contribute in accordance with her or his competence skill to
achieve the goal while respecting the functions of the other team member.
MEMBERS OF HEALTH
TEAM-
Ø Members of Community Health team includes-
Ø Medical officer
Ø Public health Nursing officer
Ø LHV
Ø ANM
Ø Radiographer, lab technician, pharmacist
etc.
Ø Medical
officer- Medical officer
is leader of community health team. He/she possess a degree of medical
education (MBBS or above). Medical officer is in-charge of community health
facility. He provides medical treatment services to the community along with
day to day administration of the community health facility.
Ø Community
health Nursing officer (CHNO or PHNO)- community health nursing officer or public health
nursing officer (in some states nomenclature is still as CHN or PHN) is a well
qualified person having a degree of
Nursing education (B.Sc. Nursing or above). Normally she/he functions at
community health center level.
Ø Functions of community health nursing
officer includes-
Ø public health nursing officer identify
health needs of community and plan community health service along with lady
health visitors.
Ø PHNO provide technical guidance to LHV and
ANM for home visiting and CNA
Ø Public health nursing officer takes
cooperation from medical officer, community leaders and other members of health
team to plans a comprehensive community
health care program for the community.
Ø She also provides direct nursing care
during home visit, school visit or clinic as per need.
Ø Public health nursing officer provides
health education to the individual, family or group for promotion of health and
prevention of illness.
Ø Public health nursing officer participates
in school health program and conduct health education sessions in schools.
Ø Public health nursing officer coordinates
nursing activities with other programs such as environmental sanitation,
national health programs, school health programs and health education programs.
Ø Public health nursing officer acts as
resource speaker for health related matters community meetings.
Ø Public health nursing officer guides and
supervises activities of lady health visitors and health assistants.
Ø Public health nursing officer acts as
trainer for trainees (medical, nursing and paramedical)who visit the health
facility.
Ø Female
health assistant or LHV-
Ø Female health assistant is a qualified and
trained LHV who undergoes 6 months special refresher course after at least 5
years service as ANM. LHV provides
supervisory services for ANMs at PHC level.
Ø Functions of female health assistant
Ø Each
LHV supervises ANMs of four-five sub centers under the working area of PHC.
Ø She guides ANMs for planning, home visiting
and reporting.
Ø She supervises ANMs and help them to improve their knowledge and
skills.
Ø Female
health worker or ANM-
Ø Female health worker is a qualified and trained
ANM who manage community health services at sub center level. She undergoes two
years ANM course after 12 years (10+2) of school education
Ø Functions of female health worker or ANM-
Ø Female health worker is responsible primary
health services at sub center level.
Ø She provides immunization services to
children up to 5 years of age
Ø She provides maternal heatlh services
including antenatal natal and postnatal care.
Ø Female health worker provides health
education for prevention of communicable diseases.
Ø She monitors growth of children up to 5
years of age and provides nutritional health education to mothers for proper
balance diet.
Ø She distributes folic acid and iron tablets
to pregnant ladies to prevent anaemia.
Ø Female health worker plan and visit homes
for survey and providing home health services and community need assessment.
Ø ANM maintains eligible couple register and
provide family planning services including distribution of contraceptives and
counselling for permanent method of family planning.
Ø She maintains record at sub centers which
includes different register for the services provided by the sub centers.
Ø ANM maintains stock of supplies of
medicines, vaccines and permanent items received at sub center.
Ø She provides treatment for minor ailments
including first aid for cuts / wounds and minor accidents.
Ø ANM coordinates with village secretary for
birth and death registration.
Ø She refers patients to PHC as per need.
Ø She prepares daily, weekly and monthly
reports and send it to PHC.
Ø She attends monthly meetings at PHC.
Ø She identifies spread of communicable
diseases and inform to higher authorities to apply disease control measures.
