प्रजनन क्षमता कम करने और जनसंख्या वृद्धि दर को धीमा करने के लक्ष्य के साथ 1952 में परिवार नियोजन कार्यक्रम शुरू करने वाला भारत दुनिया का पहला देश बन गया।
अक्टूबर 1977 के बाद से परिवार नियोजन सेवाएँ, राष्ट्रीय परिवार कल्याण कार्यक्रम के तहत प्रदान की जा रही हैं
उद्देश्य
"राष्ट्रीय अर्थव्यवस्था की आवश्यकता के अनुरूप एक स्तर पर जनसंख्या को स्थिर करने के लिए आवश्यक हद तक जन्म दर को कम करना"।

परिवार कल्याण विभाग ने उन महिलाओं की जरूरतों को पूरा करने के लिए परिवार कल्याण कार्यक्रम के कार्यान्वयन के लिए कई पायलट गतिविधियों को अंजाम दिया, जो अवांछित जन्मों और देश में को प्रजनन क्षमता धीमा करने में मदद की है।

परिवार कल्याण कार्यक्रम की गतिविधियाँ मुख्य रूप से शामिल हैं:
1. स्वस्थ प्रजनन के लिए महिलाओं के लिए परिवार नियोजन की जानकारी, परामर्श और सेवाएँ, जिसमें दंपति को सभी परिवार नियोजन के तरीकों की जानकारी शामिल है।
2. गर्भावस्था से पहले मां और बच्चे के सुरक्षित प्रसव और पोस्ट डिलीवरी और महिलाओं के उपचार के बारे में शिक्षा।
3. रोकथाम योग्य रोगों के खिलाफ शिशुओं के टीकाकरण के लिए स्वास्थ्य देखभाल।
4. यौन और प्रजनन पथ संक्रमण की रोकथाम और उपचार।

परिवार कल्याण कार्यक्रम का मूल उद्देश्य जनसंख्या को स्थिर करना और गर्भवती माता और बच्चों दोनों को टीकाकरण सहित गुणात्मक स्वास्थ्य सेवाएं प्रदान करना है। पिछले 68 वर्षों से राज्य में परिवार कल्याण कार्यक्रम केंद्र सरकार की वित्तीय सहायता से कार्यान्वित किया जा रहा है।
परिवार नियोजन कार्यक्रम अब केवल जनसंख्या स्थिरीकरण के लिए नहीं है, बल्कि प्रजनन और बाल स्वास्थ्य के लिए समग्र दृष्टिकोण में इसका बहुत बड़ा परिप्रेक्ष्य है। सेवाओं, परिवार नियोजन, टीकाकरण, प्रशिक्षण आदि में एक व्यापक स्वास्थ्य देखभाल पैकेज शामिल है। परिवार कल्याण गतिविधियों को भारत सरकार द्वारा नीतियों, दिशानिर्देशों और धन के अनुसार निर्देशित किया जाता है।

National Family Welfare Program
India became the first country in the world to initiate the family planning program in 1952 with the goal of lowering fertility and slowing the population growth rate.
Since October 1977, the services and interventions under the National Family Welfare Program
Objective
“Reducing the birth rate to the extent necessary to stabilize the population at a level consistent with the requirement of the National economy”.

The department of Family welfare undertook many pilot activities for implementation of the family welfare program to meet the needs of women who are at risk of unwanted births and has assisted the country to accelerate fertility decline.

Family Welfare Programmes activitiesMainly include:
1. Family planning information, counselling and services to women for healthy reproduction which includes information about all family planning methods to the couple.
2. Education about safe delivery and post delivery of the mother and the baby and the treatment of women before pregnancy.
3. Health care for infants immunization against preventable diseases.
4. Prevention and treatment of sexually and Reproductive Tract infection.
The basic objective of the Family welfare programme is to stabilize the population and to provide qualitative health services including immunization to both-pregnant mother and children. Since the last 68 years, the Family welfare programme is being implemented in the State with financial assistance of the central Govt.
The family planning programme is not merely intended for population stabilization now but it has much larger perspective in holistic approach towards Reproductive and child health. Services, family planning, Immunization, training etc. comprise a comprehensive health care package. The Family welfare activities are guided as per the policies, guidelines and funding by Govt. of India.

सरकार द्वारा 12 अप्रैल 2005, शुरू किया गया था। भारत की ग्रामीण आबादी, विशेष रूप से कमजोर समूहों के लिए सुलभ, सस्ती और गुणवत्तापूर्ण स्वास्थ्य देखभाल प्रदान करने के लिए। (NUHM के साथ 2013 में राष्ट्रीय स्वास्थ्य मिशन में विलय)
NRHM का मुख्य उद्देश्य ग्रामीण आबादी, विशेष रूप से कमजोर समूहों को समान, सस्ती और गुणवत्तापूर्ण स्वास्थ्य देखभाल प्रदान करना है। मिशन का जोर अंतर-क्षेत्रीय अभिसरण के साथ पूरी तरह कार्यात्मक, सामुदायिक स्वामित्व, विकेंद्रीकृत स्वास्थ्य वितरण प्रणाली की स्थापना पर है। स्तर, पानी, स्वच्छता, शिक्षा, पोषण, सामाजिक और लैंगिक एकता जैसे स्वास्थ्य के निर्धारकों की एक विस्तृत श्रृंखला पर एक साथ कार्रवाई सुनिश्चित करने के लिए।
मुख्य गतिविधियां-
• पंचायत राज संस्थाओं (पीआरआई) को प्रशिक्षित और बढ़ाने की क्षमता, सार्वजनिक स्वास्थ्य सेवाओं का स्वामित्व, नियंत्रण और प्रबंधन करना।
• महिला स्वास्थ्य कार्यकर्ता (आशा) के माध्यम से घरेलू स्तर पर बेहतर स्वास्थ्य सेवा तक पहुंच को बढ़ावा देना।
• पंचायत की ग्राम स्वास्थ्य समिति के माध्यम से प्रत्येक गांव के लिए स्वास्थ्य योजना।
• स्थानीय योजना और कार्रवाई और अधिक बहुउद्देश्यीय श्रमिकों (MPWs) को सक्षम करने के लिए एक एकीकृत फंड के माध्यम से उप-केंद्र को मजबूत करना।
• मौजूदा पीएचसी और सीएचसी को मजबूत करना और 30- 50 बेडेड का प्रावधान
• स्वस्थ जीवन शैली, तंबाकू और शराब के सेवन में कमी आदि को बढ़ावा देने के लिए सभी स्तरों पर निवारक स्वास्थ्य देखभाल के लिए क्षमता विकसित करना।
• विशेष रूप से कम सेवा वाले क्षेत्रों में गैर-लाभकारी क्षेत्र को बढ़ावा देना।
पूरक रणनीतियाँ
• नागरिकों को उचित लागत पर गुणवत्तापूर्ण सेवा की उपलब्धता सुनिश्चित करने के लिए अनौपचारिक ग्रामीण चिकित्सकों सहित निजी क्षेत्र का विनियमन।
• सार्वजनिक स्वास्थ्य लक्ष्यों को प्राप्त करने के लिए सार्वजनिक निजी भागीदारी को बढ़ावा देना।
• मेनस्ट्रीमिंग आयुष - स्थानीय स्वास्थ्य परंपराओं को पुनर्जीवित करना।
• ग्राम स्वास्थ्य और स्वच्छता समिति (पंचायत प्रतिनिधि / एस, एएनएम / एमपीडब्ल्यू, आंगनवाड़ी कार्यकर्ता, शिक्षक, आशा, सामुदायिक स्वास्थ्य स्वयंसेवकों से मिलकर ग्राम स्तर पर) के लिए काम करना
• जिला स्वास्थ्य मिशन, जिला स्वास्थ्य प्रमुख के नेतृत्व में संयोजक के रूप में जिला स्वास्थ्य प्रमुख और सभी संबंधित विभागों, गैर सरकारी संगठनों, निजी पेशेवरों आदि के नेतृत्व काम करना

My Student Support System

 National Rural Health Mission (NRHM) was launched by Govt. of India 12th April 2005, to provide accessible, affordable and quality health care to the rural population, especially the vulnerable groups. ( merged with national health mission in 2013 along with NUHM)
Main objective of NRHM is to provide equitable, affordable and quality health care to the rural population, especially the vulnerable groups.The thrust of the mission is on establishing a fully functional, community owned, decentralized health delivery system with inter-sectoral convergence at all levels, to ensure simultaneous action on a wide range of determinants of health such as water, sanitation, education, nutrition, social and gender equality.
community and facility-based care and strengthening referrals between various levels of health care system to create a continuous care pathway is also to be focussed.
Main activities-
  • Train and enhance capacity of Panchayat Raj Institutions (PRIs) to own, control and manage public health services.
  • Promote access to improved healthcare at household level through the female health activist (ASHA).
  • Health Plan for each village through Village Health Committee of the Panchayat.
  • Strengthening sub - centre through an untied fund to enable local planning and action and more Multi-Purpose Workers (MPWs).
  • Strengthening existing PHCs and CHCs, and provision of 30- 50 bedded
  • Developing capacities for preventive health care at all levels for promoting healthy life styles, reduction in consumption of tobacco and alcohol etc.
  • Promoting non-profit sector particularly in under-served areas.
Supplementary Strategies
  • Regulation of Private Sector including the informal rural practitioners to ensure availability of quality service to citizens at reasonable cost.
  • Promotion of Public Private Partnerships for achieving public health goals.
  • Mainstreaming AYUSH – revitalizing local health traditions.
Institutional Mechanisms
  • Village Health & Sanitation Samiti (at village level consisting of Panchayat Representative/s, ANM/MPW, Anganwadi worker, teacher, ASHA, community health volunteers
  • Rogi Kalyan Samiti (or equivalent) for community management of public hospitals
  • District Health Mission, under the leadership of Zila Parishad with District Health Head as Convener and all relevant departments, NGOs, private professionals etc represented on it

RCH - ENGLISH


Reproductive and child health program was launched in 1997. It is defined as “People have the ability to reproduce and regulate their fertility; women are able to go through pregnancy and childbirth safely, the outcome of pregnancy is successful in terms of maternal and infant survival and well being; and couples are able to have sexual relations free of the fear of pregnancy and contract diseases”
Aim
• To bring down the birth rate below 21 per 1000 population,
• To reduce the infant mortality rate below 60 per 1000 live birth and
• To bring down the maternal mortality rate
• 80%% institutional delivery, 100% antenata l care and 100% immunization of children were other targeted aims of the RCH programme
Activities
The RCH programme is envisaged to provide an integrated package of services, which will include the following:
1.     Services for mothers during pregnancy, child birth and post-natal period, and also safe abortion services, whenever required.
2.     Services for children like newborn care, immunization, Vitamin A prophylaxis, Oral Rehydration Therapy (ORT) for diarrhoea, management of Acute Respiratory Infections (ARI), anaemia control etc.
3.     Services for eligible couples through availability and promotion of use of contraceptive methods, and infertility services when required.
4.     Prevention and management of Reproductive Tract Infections (RTIs) and Sexually Transmitted Infections (STIs).
5.     Adolescent health services including counselling of family life and reproductive health.

RCH – II

Reproductive and child health program phase II  was launched in 2005.
Aim
• To bring down maternal mortality and morbidity.
• To bring down child mortality and morbidity
• special emphasis on rural health care.
Activities
The activities and strategy under RCH programme phase II includes
1.     Essential obstetric care
• Promotion of institutional deliveries – 50% of the PHCs and CHCs made operational as 24 hours delivery centers.
• Skilled attendance at birth
• Policy descions to permit Health workers to use drugs in emergency situations to reduce maternal mortality
2.     Emergency obstetric care
• Operationalisation of FRUs to provide: – 24 hours delivery services
– Emergency obstetric care
– New born care and emergency care of the sick child
– Full range of family planning services
– Safe abortion services
– Treatment of RTI and STI
 – Blood storage facility
 – Essential laboratory services
– Referral ( transport ) services
3.     New initiatives
• Training of PHC doctors in life saving anesthetic skills for emergency obstetric care a FRUs
• Setting up of blood storage centers at FRUs
• Janani Suraksha Yojana (JSY)

RCH - HINDI


आरसीएच - 1
प्रजनन और बाल स्वास्थ्य कार्यक्रम 1997 में शुरू किया गया था। इसे "लोगों में प्रजनन क्षमता को पुन: उत्पन्न करने और विनियमित करने की क्षमता के रूप में परिभाषित किया गया है; महिलाएं गर्भावस्था और प्रसव के दौरान सुरक्षित रूप से गुजरने में सक्षम हैं, गर्भावस्था का परिणाम मातृ और शिशु के जीवित रहने और कल्याण के संदर्भ में सफल होता है; और जोड़े गर्भावस्था और अनुबंध रोगों के डरके बग़ैर यौन संबंध बनाने में सक्षम हैं ”
लक्ष्य
• प्रति 1000 जनसंख्या पर जन्म दर 21 से नीचे लाने के लिए,
• प्रति 1000 जीवित जन्म पर 60 से कम शिशु मृत्यु दर को कम करने के लिए और
• मातृ मृत्यु दर में कमी लाना
• 80 %% संस्थागत प्रसव, 100% antenatal केयर और 100% बच्चों का टीकाकरण RCH कार्यक्रम के अन्य लक्षित उद्देश्य थे
क्रियाएँ
RCH कार्यक्रम को सेवाओं का एक एकीकृत पैकेज प्रदान करने की परिकल्पना की गई है, जिसमें निम्नलिखित शामिल होंगे:
1. गर्भावस्था के दौरान माताओं के लिए सेवाएँ, प्रसव के बाद और जन्म के बाद की अवधि, और जब भी आवश्यक हो, सुरक्षित गर्भपात सेवाएँ।

5. परिवार के जीवन और प्रजनन स्वास्थ्य की परामर्श सहित किशोर स्वास्थ्य सेवाएं।
  
आरसीएच - 2
प्रजनन और बाल स्वास्थ्य कार्यक्रम चरण 2, 2005 में शुरू किया गया था।
लक्ष्य
• मातृ मृत्यु दर और रुग्णता को कम करने के लिए।
• बाल मृत्यु दर और रुग्णता को कम करने के लिए
• ग्रामीण स्वास्थ्य देखभाल पर विशेष जोर।
क्रियाएँ
आरसीएच कार्यक्रम चरण II के तहत गतिविधियों और रणनीति में शामिल हैं
1. आवश्यक प्रसूति देखभाल
• संस्थागत प्रसव को बढ़ावा देना - पीएचसी और सीएचसी का 50% 24 घंटे वितरण केंद्र के रूप में परिचालन में आया।
• जन्म के समय कुशल उपस्थिति
• मातृ मृत्यु दर को कम करने के लिए स्वास्थ्य कार्यकर्ताओं को आपातकालीन स्थितियों में दवाओं का उपयोग करने की अनुमति देने के लिए नीतिगत अवतरण
2. आपातकालीन प्रसूति देखभाल
• प्रदान करने के लिए FRUs का संचालन: - 24 घंटे वितरण सेवाएं
- आपातकालीन प्रसूति देखभाल
- बीमार बच्चे की जन्मजात देखभाल और आपातकालीन देखभाल
- परिवार नियोजन सेवाओं की पूरी श्रृंखला
- सुरक्षित गर्भपात सेवाएं
- आरटीआई और एसटीआई का उपचार
 - रक्त भंडारण की सुविधा
 - आवश्यक प्रयोगशाला सेवाएं
- रेफरल (परिवहन) सेवाएं
3. नई पहल
• आपातकालीन प्रसूति देखभाल के लिए एनेस्थेटिक कौशल को बचाने वाले जीवन में पीएचसी डॉक्टरों का प्रशिक्षण एक एफआरयू
• एफआरयू में रक्त भंडारण केंद्र स्थापित करना

यूनिवर्सल इम्यूनाइजेशन प्रोग्राम
भारत में टीकाकरण कार्यक्रम 1978 में स्वास्थ्य और परिवार कल्याण मंत्रालय, भारत सरकार द्वारा  विस्तारित कार्यक्रम टीकाकरण ’(EPI) के रूप में पेश किया गया था। 1985 में, इस कार्यक्रम को 'यूनिवर्सल इम्यूनाइजेशन प्रोग्राम' (UIP) के रूप में संशोधित किया गया था, जिसे चरणबद्ध तरीके से देश के सभी जिलों में 1989-90 तक दुनिया के सबसे बड़े स्वास्थ्य कार्यक्रम में से एक के रूप में लागू किया गया था।
उद्देश्य
इस कार्यक्रम का उद्देश्य बच्चों को टीके से बचाव योग्य बीमारियों से 100 प्रतिशत कवरेज से रोकना है।
रणनीति और गतिविधिय:
इस कार्यक्रम के तहत गतिविधियों को एक उपयुक्त स्वास्थ्य प्रणाली के माध्यम से स्वास्थ्य के स्वीकार्य, सस्ती और टिकाऊ मानक प्राप्त करने की दिशा में निर्देशित किया जाता है। वैक्सीन रोके जाने वाले रोगों के खिलाफ बच्चों के सार्वभौमिक टीकाकरण का प्रावधान इस नीति के तहत प्रमुख लक्ष्यों में से एक है। 
कोल्ड चेन का रखरखाव इस कार्यक्रम का बहुत महत्वपूर्ण घटक है। कोल्ड चेन निर्माण के बिंदु से अनुशंसित तापमान सीमा पर टीके के भंडारण और परिवहन की व्यवस्था है। भारत ने यह सुनिश्चित करने के लिए एक विशाल कोल्ड चेन इन्फ्रास्ट्रक्चर का निर्माण किया है कि देश भर में लाखों लाभार्थियों तक केवल शक्तिशाली और प्रभावी टीके ही पहुँचें
एक अन्य महत्वपूर्ण गतिविधि सुरक्षित इंजेक्शन अभ्यास है। सुरक्षित इंजेक्शन प्रथाओं को सुनिश्चित करने के लिए, भारत सरकार इंजेक्शन सुरक्षा उपकरणों (एडी सिरिंज, पुनर्गठन सीरिंज, हब कटर और अपशिष्ट निपटान बैग) की निरंतर आपूर्ति सुनिश्चित करने का प्रयास करती है। नौकरी प्रशिक्षण (सहायक पर्यवेक्षण) पर नौकरी-सहायता द्वारा प्रशिक्षण आयोजित और समर्थित हैं।
अगली गतिविधि AEFI निगरानी है- AEFI निगरानी टीकाकरण सुरक्षा की निगरानी करती है, टीकाकरण के बाद प्रतिकूल घटनाओं का पता लगाती है और उसका जवाब देती है; असुरक्षित टीकाकरण प्रथाओं को सही करता है, स्वास्थ्य पर घटना के नकारात्मक प्रभाव को कम करता है और टीकाकरण गतिविधियों की गुणवत्ता में योगदान देता है।
रणनीतिक संचार से तात्पर्य नीति-निर्माण और सुसंगत सूचना के माध्यम से सुसंगत सूचना गतिविधि के लिए मार्गदर्शन से है। मीडिया वकालत, सक्रिय योजना और प्रभावी मीडिया प्रतिक्रिया का मुद्दा देश में पूर्ण रूटीन टीकाकरण कवरेज प्राप्त करने के लिए रणनीतिक संचार समर्थन के प्रमुख तत्वों में से एक के रूप में उभर रहा है।
इस कार्यक्रम के तहत टीके और कोल्ड चेन हैंडलर का प्रशिक्षण भी एक महत्वपूर्ण गतिविधि है।

Universal Immunisation Programme

Immunization Programme in India was introduced in 1978 as ‘Expanded Programme of Immunization’ (EPI) by the Ministry of Health and Family Welfare, Government of India. In 1985, the programme was modified as ‘Universal Immunization Programme’ (UIP) to be implemented in phased manner to cover all districts in the country by 1989-90 with the one of largest health programme in the world.
Objective
Objective of this program is to  prevent children from vaccine preventable diseases by 100 percent coverage .

Strategy and policy:
Activities under this program are directed towards achieving an acceptable, affordable and sustainable standard of health through an appropriate health system. Provision of universal immunization of children against vaccine preventable diseases is one of the major goals under this policy. Country’s five year plan also puts emphasis on reduction in maternal and infant mortality rates as major maternal and child health indicators.
Maintenance of cold chain is very important component of this program. Cold Chain is a system of storing and transporting vaccine at the recommended temperature range from the point of manufacture to point of use. India has built a vast cold chain infrastructure to ensure that only potent and effective vaccines reach millions of beneficiaries across the country
Another important activity is safe injection practice. To ensure safe injection practices, Government of India endeavours to ensure continuous supply of injection safety equipments (AD syringes, reconstitution syringes, hub cutters and waste disposal bags). Trainings are conducted and supported by job-aids, on job training (supportive supervision).
Next activity is AEFI surveillance- AEFI surveillance monitors immunization safety, detects and responds to adverse events following immunization; corrects unsafe immunization practices, reduces the negative impact of the event on health and contributes to the quality of immunization activities.
Strategic Communication refers to policy-making and guidance for consistent information activity through coherent messaging. The issue of media advocacy, proactive planning and effective media response is emerging as one of the key elements of strategic communication support to achieving full Routine Immunization coverage in the country.
Training of vaccinators and cold chain handlers is also an important activity under this program.
