HYPERTENSION-

•        रक्तचाप (बीपी) को धमनी की दीवार के खिलाफ लगाए गए रक्त के बल के रूप में परिभाषित किया गया है। यह रक्त की VISCOSITY (THICKNESS) और रक्त वाहिका के  Resistance को दर्शाता है। रक्तचाप के लिए दो शब्दों का प्रयोग किया जाता है-

•        सिस्टोलिक और डायस्टोलिक

•        सिस्टोलिक रक्तचाप उच्चतम धमनी दबाव होता है जब हृदय सिकुड़ता है और रक्त को धमनियों में धकेलता है।

•        डायस्टोलिक ब्लड प्रेशर सबसे कम धमनी दबाव होता है जब दिल रक्त से भरने के लिए आराम करता है और रक्त को धमनियों में पंप नहीं करता है।

•        उच्च रक्तचाप को 140 मिमी एचजी से अधिक सिस्टोलिक रक्तचाप और 90 मिमी एचजी से अधिक डायस्टोलिक दबाव के रूप में परिभाषित किया जाता है, जो स्वास्थ्य देखभाल प्रदाता के साथ दो या दो से अधिक संपर्कों के दौरान लिए गए दो या अधिक सटीक रक्तचाप माप के आधार पर होता है।

•        यह रक्तचाप के लिए संदर्भ सीमा है

•        Normal -120 and 80

•        Prehypertension-  120–139 and 80–89

•        Stage 1 hypertension-  140–159 or 90–99

•        Stage 2 hypertension-  160 or 100

EFFECTS OF HYPERTENSION-

PATHOPHYSIOLOGY OF HYPERTENSION-

•        उच्च रक्तचाप कार्डियक आउटपुट में वृद्धि या परिधीय प्रतिरोध में वृद्धि या दोनों के परिणामस्वरूप हो सकता है। यह समझा जाता है कि उच्च रक्तचाप एक multifactorial condition है। 

CAUSATIVE FACTORS OF HYPERTENSION-

•        उच्च रक्तचाप के कारणों के रूप में कई कारकों की पहचान की गई है: •Increased sympathetic nervous system activity related to dysfunction of the autonomic nervous system

•        • सोडियम, क्लोराइड, और पानी के वृक्कीय पुनःअवशोषण में वृद्धि (overloading of vascular system)  यह परिधीय संवहनी प्रतिरोध (peripheral vascular resistance)  का कारण बनता है है और अंत में उच्च रक्तचाप में योगदान देता है।

•        रेनिन-एंजियोटेंसिन-एल्डोस्टेरोन प्रणाली की बढ़ी हुई गतिविधि, जिसके परिणामस्वरूप बाह्य तरल पदार्थ की मात्रा का विस्तार होता है और प्रणालीगत संवहनी प्रतिरोध (systemic vascular resistance)  में वृद्धि होती है जिससे उच्च रक्तचाप होता है

•        संवहनी एंडोथेलियम की शिथिलता या एथेरोस्क्लेरोसिस से संबंधित धमनी के VASODILATATION में कमी।

•        • इंसुलिन क्रिया का प्रतिरोध, जो उच्च रक्तचाप, टाइप 2 मधुमेह मेलिटस, हाइपरट्रिग्लिसराइडेमिया, मोटापा, और ग्लूकोज असहिष्णुता को जोड़ने वाला एक सामान्य कारक हो सकता है

•        हृदय और रक्त वाहिकाओं में संरचनात्मक और कार्यात्मक परिवर्तन उम्र बढ़ने के साथ होने वाले रक्तचाप में वृद्धि में योगदान करते हैं। इन परिवर्तनों का परिणाम प्रमुख रक्त वाहिकाओं की लोच में कमी है।

CLINICAL MANIFESTATIONS-

•        प्रारंभ में उच्च रक्तचाप asymptomatic होता है, शारीरिक परीक्षण से उच्च रक्तचाप के अलावा कोई असामान्यता प्रकट नहीं हो सकती है, इसलिए इसे साइलेंट किलर के रूप में जाना जाता है।

•        बाद में रेटिनल परिवर्तन जैसे कि रक्तस्राव, एक्सयूडेट्स (fluid collection), धमनियों का संकुचन, और cotton wool spot (छोटे छोटे infarctions) होते हैं। गंभीर उच्च रक्तचाप में, पैपिल्डेमा (ऑप्टिक डिस्क की सूजन) देखी जा सकती है।

•        जब विशिष्ट संकेत और लक्षण दिखाई देते हैं, तो वे आमतौर पर संवहनी क्षति का संकेत देते हैं, जिसमें शामिल वाहिकाओं द्वारा सेवा किए जाने वाले अंगों से संबंधित विशिष्ट अभिव्यक्तियाँ होती हैं। एनजाइना और मायोकार्डियल इन्फ्रक्शन (MI) के साथ कोरोनरी धमनी की बीमारी उच्च रक्तचाप के सामान्य परिणाम हैं।

DIAGNOSTIC INVESTIGATIONS-

•        उच्च रक्तचाप का आकलन करने के लिए एक संपूर्ण स्वास्थ्य इतिहास और शारीरिक परीक्षण और रक्तचाप की निगरानी आवश्यक है।

•        संभावित लक्ष्य अंग क्षति का आकलन करने के लिए रेटिना की जांच की जाती है और प्रयोगशाला INVESTIGATIONS किया जाता है।

•        नियमित प्रयोगशाला परीक्षणों में यूरिनलिसिस, रक्त रसायन जैसे सोडियम, पोटेशियम, क्रिएटिनिन, उपवास ग्लूकोज और कुल और उच्च घनत्व वाले लिपोप्रोटीन [एचडीएल] कोलेस्ट्रॉल के स्तर (लिपिड प्रोफाइल) का विश्लेषण शामिल है।

•        हृदय पर प्रभाव का आकलन करने के लिए 12-लीड इलेक्ट्रोकार्डियोग्राम (ईसीजी) बहुत उपयोगी नैदानिक ​​प्रक्रिया है। और

•         इकोकार्डियोग्राफी द्वारा बाएं निलय अतिवृद्धि का आकलन किया जा सकता है

•         

MANAGEMENT OF HYPERTENSION-

•        उच्च रक्तचाप के उपचार का मूल लक्ष्य 140/90 मिमी एचजी या उससे कम धमनी रक्तचाप को प्राप्त करके और बनाए रखकर जटिलताओं और मृत्यु को रोकना है।

HYPERTENSION-

•        Blood pressure (BP) is defined as the force of blood exerted against the arterial wall. It reflects the blood viscosity (thickness) and resistance of blood vessel.  Two terms are used for blood pressure-

•        Systolic and Diastolic

•        Systolic blood pressure  is the highest arterial pressure when the heart contracts and pushes blood into arteries.

•        Diastolic blood pressure is the lowest arterial pressure when the heart relaxes to fill with blood and not pumping blood into arteries.

•        Hypertension is defined as a systolic blood pressure greater than 140 mm Hg and a diastolic pressure greater than 90 mm Hg based on the average of two or more accurate blood pressure measurements taken during two or more contacts with a health care provider

•        This is the reference  range for blood pressure

•        Normal -120 and 80

•        Prehypertension-  120–139 and 80–89

•        Stage 1 hypertension-  140–159 or 90–99

•        Stage 2 hypertension-  160 or 100

EFFECTS OF HYPERTENSION-

•        Hypertension is sometimes called the “silent killer” because people who have it are often symptom free. Once identified, elevated blood pressure should be monitored at regular intervals because hypertension is a lifelong condition.

•        Prolonged blood pressure elevation eventually damages blood vessels throughout the body, particularly in target organs such as the heart, kidneys, brain, and eyes.

•        uncontrolled and longterm hypertension normally results in myocardial infarction, heart failure, renal failure, strokes, and impaired vision.

PATHOPHYSIOLOGY OF HYPERTENSION-

•        Hypertension can result from an increase in cardiac output or an increase in peripheral resistance  or both. it is understood that hypertension is a multifactorial condition.

CAUSATIVE FACTORS OF HYPERTENSION-

•        Many factors have been identified as causes of hypertension:

•        •Increased sympathetic nervous system activity related to dysfunction of the autonomic nervous system

•        • Increased renal reabsorption of sodium, chloride, and water  (overloading of vascular system) . This leads to peripheral vascular resistance and finally contributing to hypertension.

•          • Increased activity of the renin–angiotensin–aldosterone system, resulting in expansion of extracellular fluid volume and increased systemic vascular resistance  leading to hypertension

•        Decreased vasodilation of the arterioles related to dysfunction of the vascular endothelium or atherosclerosis.

•        • Resistance to insulin action, which may be a common factor linking hypertension, type 2 diabetes mellitus, hypertriglyceridemia, obesity, and glucose intolerance

•        Structural and functional changes in the heart and blood vessels contribute to increases in blood pressure that occur with aging. The result of these changes is decreased elasticity of the major blood vessels.

CLINICAL MANIFESTATIONS-

•        Initially hypertension is asymptomatic, Physical examination may reveal no abnormalities other than elevated blood pressure that’s why it is known as silent killer.

•        Later on retinal changes such as hemorrhages, exudates (fluid accumulation), arteriolar narrowing, and cotton-wool spots (small infarctions) occur. In severe hypertension, papilledema (swelling of the optic disk) may be seen.

•        When specific signs and symptoms appear, they usually indicate vascular damage, with specific manifestations related to the organs served by the involved vessels. Coronary artery disease with angina and myocardial infarction are common consequences of hypertension.

•        Cerebrovascular involvement may lead to a stroke or transient ischemic attack (TIA), manifested by alterations in vision or speech, dizziness, weakness, a sudden fall, or transient or permanent paralysis on one side (hemiplegia).

DIAGNOSTIC INVESTIGATIONS-

•        To assess hypertension a thorough health history and physical examination & BP monitoring are necessary.

•        The retinas are examined and laboratory studies are performed to assess possible target organ damage.

•        Routine laboratory tests include urinalysis, blood chemistry  such as analysis of sodium, potassium, creatinine, fasting glucose, and total and high-density lipoprotein [HDL] cholesterol levels  (lipid profile)

•        A 12-lead electrocardiogram (ECG) is very useful diagnostic procedure to assess effects on heart . and

•        Left ventricular hypertrophy can be assessed by echocardiography

MANAGEMENT OF HYPERTENSION-

•        The basic goal of hypertension treatment is to prevent complications and death by achieving and maintaining the arterial blood pressure at 140/90 mm Hg or lower. 

•        Research findings demonstrate that weight loss, reduced alcohol and sodium intake, and regular physical activity are very important along with medications under guidance of a cardiologist, to reduce and manage blood pressure . Regular follow-up is necessary.

Maslow’s hierarchy of needs and motivations

 

•       मास्लो के अनुसार, मनोविकृति विज्ञान का प्राथमिक कारण किसी की मूलभूत आवश्यकताओं को पूरा करने में विफलता है। 1954 में मास्लो ने इन जरूरतों को अपने मॉडल के भीतर समझाया जो सबसे बुनियादी जरूरतों से लेकर उच्च स्तर की जरूरतों के क्रम में प्रस्तुत किए गए हैं

•       मास्लो के जरूरतों और प्रेरणा के मॉडल को मास्लो के जरूरतों और प्रेरणा के पदानुक्रम के त्रिकोण hierarchy triangle के रूप में भी जाना जाता है।

 

PHYSIOLOGICAL NEEDS

•       ये ज़रूरतें मनुष्य की बुनियादी ज़रूरतें हैं और इन्हें जीवित रहने की ज़रूरतों के रूप में भी जाना जाता है क्योंकि ये जीवित रहने के लिए आवश्यक हैं। ये जरूरतें हैं हवा, पानी और भोजन।

 

SAFETY NEEDS

•       जब Physiological जरूरतें पूरी हो जाती हैं, तो सुरक्षा और संरक्षा की जरूरत पैदा होती है। इस स्तर में सुरक्षा, स्थिरता और चिंता से मुक्ति जैसी ज़रूरतें शामिल हैं। इस जरूरत को पूरा करने के लिए एक व्यक्ति आश्रय या घर की तलाश करता है।

•       इस स्तर तक हमारे जीवन के बारे में विचार बहुत संकीर्ण है। एक व्यक्ति व्यापक रूप से नहीं सोच सकता है / वह केवल "रोटी, कपड़ा और मकान सहित हवा और पानी" के बारे में सोचता है।

 

SOCIAL NEEDS-

ESTEEM NEEDS-

•       इसमें दिन-प्रतिदिन के आधार पर अपने आसपास की दुनिया के भीतर की सुंदरता की सराहना करने की क्षमता शामिल होगी। मास्लो के सिद्धांतों के अनुसार lower  जरूरतों को पूरा करने के बाद एक व्यक्ति को कला और प्रकृति में सौंदर्य, जीवन में समरूपता, संतुलन और व्यवस्था की आवश्यकता होती है।

 

SELF ACTUALIZATION-

•       मैस्लो के अनुसार सभी आवश्यकताओं की पूर्ति के बाद व्यक्ति को आत्म-प्राप्ति, आत्म-साक्षात्कार, आत्म-विकास और वह सब कुछ प्राप्त करने की आवश्यकता होती है जो एक व्यक्ति सक्षम है। इस स्तर पर व्यक्ति जीवन को बुद्धि विवेक और स्वीकृति के साथ बहुत व्यापक दृष्टि से देखता है।

•       आवश्यकता के इस स्तर पर व्यक्ति जीवन के दार्शनिक पक्ष में अधिक रूचि लेता है। कुछ मनोवैज्ञानिक इसे आध्यात्मिक आवश्यकता कहते हैं।

Maslow’s hierarchy of needs and motivations

 

•       According to Maslow, the primary cause of psychopathology is the failure to gratify one’s fundamental needs. In 1954 Maslow explained these needs within his model which are presented in order from the most basic needs to higher level needs

•       Maslow’s model of needs and motivation is also known as Maslow’s triangle of hierarchy of needs and motivation.

 

PHYSIOLOGICAL NEEDS

•       These needs are basic needs of human being and also known as survival needs as they are required for survival. These needs are air, water and food.

 

SAFETY NEEDS

•       when physiological needs have been satisfied, the need for safety and security arises. This level includes needs such as protection, stability and freedom from anxiety. To satisfy this need a person seeks a shelter or a home.

•       Up to this level our idea of life is very narrow . A person can’t think in broad way he/she thinks only about “ Roti , kapda and makaan including air and water.

 

SOCIAL NEEDS-

•       Social needs are also known as love and belonging needs. when physiological and safety needs have been met, the person begin to feel the need for friends, a partner, children, affectionate relationships in general and even a sense of community

•       These needs manifest themselves in our desire to marry, have a family or be part of a community or club. Failure to satisfy these needs can lead to feelings of loneliness and anxiety.

 

ESTEEM NEEDS-

•       This includes need for self respect and self esteem. Maslow distinguished between two levels of esteem needs. The lower level is the need for the respect of others, the need for status, recognition, appreciation and dignity. 

•       The higher level involves the need for self-respect and includes feelings of confidence, satisfaction, achievement, independence and freedom.

 

COGNITIVE NEEDS-

•       Maslow believed that humans have the need to increase their intelligence and thereby chase knowledge. Cognitive needs is the expression of the natural human need to learn, explore, discover and create to get a better understanding of the world.   

•       This includes need for enhancement of knowledge and understanding, curiosity, exploration, need for meaning and predictability.

 

AESTHETIC NEEDS

•       This would consist of having the ability to appreciate the beauty within the world around one's self, on a day-to-day basis. According to Maslow's theories after satisfying lower needs a person needs for Beauty in art and nature, symmetry, balance and order in life.

 

SELF ACTUALIZATION-

•       According to Maslow after satisfying all needs the client needs for self actualization, self realization , self development and becoming all that a person is capable of. At this level the person perceive life with very broad view with intelligence wisdom and acceptance.

At this level of need the person is more interested in philosophical aspect of life. Some psychologists call it spiritual need.
