Types of supervision-

Ø Supervision is of two types

1.    Direct supervision

2.    Indirect supervision

Direct supervision-

This is done through face to face talk with the workers. This can be exercised at the ward level in the hospital, PHC, Sub center and in the community setting. 

Ø The following considerations are essential in direct supervision:

        Do not loose temper or abuse.

        Use democratic approach and avoid autocratic methods.

        Reprimand if necessary in private and do it promptly.

        Give workers a chance to reply

        Do not talk too much and too fast.

        Be human in behavior.

        Do not take it granted that the worker has understood everything told to him.

        Do not give instructions in a haphazard way.

Indirect supervision-

        It is done with the help of records and reports of the workers and through written instructions or through some agency between the worker and the supervisor. 

        The examples of indirect supervision in ward setting will include- patient record and reports, case sheets, general order book, round book, medicine ledger books, call books, procedure manuals, and nurses’ notes, nursing care plans and other records and reports pertaining to the institution.

        The examples of community health setting include- tour diary schedule for each worker, work diary, performance reports, MCH Cards, Eligible couple register, monthly reports and other records and reports pertaining to the health center.

        Indirect supervision involves-

        Ensuring that every worker is carrying out allotted work in accordance with the plan of operation and with prescribed methodology and in keeping pace with the time as for as possible.

        Analyzing the monthly progress reports to know the inputs of efforts and the achievements of the worker and their relations with each other.

        Analyzing what amount of work allotted for the month has been done with reasons for non-performance and providing suitable guidance for the same.

Steps of supervision-

        Steps to be followed during supervision are as given below:-

1.    Definition of the job to be done.

2.    Selection and organization of supervisor’s activities.

3.    Anticipation of difficulties.

4.    Establishment of criteria for evaluation.

Definition of the job to be done.

 It refers to that the supervisor should know his/her position clearly whether he/she is in line or staff position. The line position is of administrative nature and the staff position is advisory and consultative. 

The definition of job requires  understanding of the general and specific objectives of the job. The supervisor should also be aware of problems and needs of the community as well as qualifications, qualities and problems of the personals.

Selection and organization of supervisor’s activities: -

It refers to the activities chosen should be compatible with the objectives of the supervision and should be based on the available facilities, personnel relating to service and also should be based on the expected outcome by the agency and the community.

Anticipation of difficulties -

It means anticipating difficulties according to the type of job, i.e., service problems, administrative difficulties, personal factor (for example, resources availbility, poor relations, emergency task, etc.) and other related problems.

Establishment of criteria for evaluation: -

Evaluation refers to determining as to what extent the programme has met goals or objectives according to the plan. This can be done by objective measurement, subjective comparison of service indices and opinion or judgment of the staff, supervisor and others.

प्रसव के तृतीय चरण का प्रबंधन (Management of Third Stage of Labor)

परिचय

  • प्रसव का तृतीय चरण अत्यंत महत्वपूर्ण होता है।

  • सामान्य प्रथम और द्वितीय चरण भी अचानक असामान्य बन सकते हैं और गंभीर जटिलताएँ हो सकती हैं।

  • इस चरण के प्रबंधन का मुख्य उद्देश्य है –

    • सतर्क निगरानी रखना

    • निर्धारित दिशा-निर्देशों का पालन करना

    • जटिलताओं की रोकथाम करना (विशेषकर Postpartum Hemorrhage)


प्रबंधन की विधियाँ

तृतीय चरण के प्रबंधन की दो विधियाँ प्रचलित हैं –

  1. Expectant Management (प्रतीक्षा आधारित प्रबंधन)

  2. Active Management (सक्रिय प्रबंधन – पसंदीदा)


1. Expectant Management

  • इसमें प्लेसेंटा (अवस्थापना) का अलग होना और योनि में नीचे आना स्वाभाविक रूप से होने दिया जाता है।

  • आवश्यकता पड़ने पर केवल न्यूनतम सहायता दी जाती है।

  • रोगी को कभी अकेला न छोड़ा जाए, निरंतर निगरानी की जाए।

  • महिला को dorsal recumbent position में रखा जाए।

Fundus palpation (गर्भाशय फंडस का स्पर्श परीक्षण):

  • (a) प्लेसेंटा के अलग होने के लक्षण पहचानने के लिए

  • (b) गर्भाशय की गतिविधि (संकुचन और शिथिलता) जानने के लिए

  • (c) फंडस की "cupping" पहचानने के लिए (गर्भाशय उलटने का प्रारंभिक लक्षण)

Placenta separation के लक्षण:

  • गर्भाशय का संकुचन

  • प्रसव पीड़ा जैसी भावना

  • रक्त का फव्वारा (10–20 सेकंड में बंद हो जाना)

  • नाल (cord) का लंबा होना

Placenta निकालना:

  • जैसे ही अलग होने के लक्षण दिखें, महिला को जोर लगाने के लिए कहा जाए।

  • प्रायः महिला हल्के प्रयास से ही प्लेसेंटा बाहर निकाल देती है।

  • प्लेसेंटा बाहर आते ही उसे पकड़कर धीरे-धीरे घुमाया जाता है ताकि झिल्ली (membranes) पूरी निकल आएँ।

  • यदि झिल्ली फटने लगे तो उसे स्पॉन्ज-होल्डिंग फोर्सेप्स से पकड़कर घुमाते हुए बाहर निकाला जाता है।

  • यदि स्वतः निष्कासन न हो तो bimanual technique से गर्भाशय को ऊपर और पीछे (नाभि की ओर) धकेलते हुए प्लेसेंटा बाहर निकालने में सहायता की जाती है।


2. Active Management (AMTSL – Preferred)

  • WHO के अनुसार, प्रसव के 1 मिनट के भीतर parenteral oxytocic देकर शक्तिशाली गर्भाशय संकुचन उत्पन्न करना इसका सिद्धांत है।

  • इससे प्लेसेंटा जल्दी और सुरक्षित रूप से अलग होकर बाहर आ जाता है।

लाभ:

  • रक्तस्राव (blood loss) को लगभग पाँचवें हिस्से तक कम कर देता है।

  • तृतीय चरण की अवधि को आधा कर देता है।

हानि:

  • Retained placenta (1–2%) की संभावना थोड़ी बढ़ जाती है।

प्रक्रिया:

  • प्रसव के तुरंत बाद Injection Oxytocin 10 units IM (पसंदीदा) या Methergine 0.2 mg IM दिया जाता है।

  • यदि प्लेसेंटा अपने-आप न निकले तो Controlled Cord Traction (Brandt-Andrews method) से निकाला जाता है।

  • पहली कोशिश विफल होने पर 2–3 मिनट बाद पुनः प्रयास किया जाता है, फिर 10 मिनट पर।

  • विफल रहने पर manual removal किया जाता है।


Placenta, Membranes और Cord की जाँच

Placenta:

  • ट्रे में रखकर बहते पानी से खून और थक्के साफ करें।

  • Maternal surface की जाँच करें – यह ग्रे रंग की decidua से ढकी होती है।

  • देखें कि सतह पूर्ण है या कहीं कोई टुकड़ा गायब तो नहीं।

  • अतिरिक्त लोब (succenturiate lobe), infarct, hemorrhage, tumor या नोड्यूल देखें।

  • प्लेसेंटा का वजन दर्ज करें।

Membranes:

  • Chorion और Amnion की पूर्णता की जाँच करें।

  • असामान्य रक्त वाहिनियाँ (abnormal vessels) देखें।

Umbilical Cord (नाल):

  • वाहिकाओं (vessels) की गिनती करें – सामान्यतः 2 धमनियाँ + 1 शिरा

  • नाल का insertion site देखें।

  • सच्चे knots, thrombi या constrictions की जाँच करें।

प्रसव के द्वितीय चरण का प्रबंधन (Management of Second Stage of Labor)

प्रथम चरण से द्वितीय चरण में परिवर्तन की पहचान

  • गर्भाशय संकुचनों की तीव्रता बढ़ना

  • Bearing-down efforts (धक्का लगाने की इच्छा)

  • प्रस्तुत भाग के नीचे आने पर शौच जैसा जोर लगाने की प्रवृत्ति

  • गर्भाशय ग्रीवा का पूर्ण प्रसार (Vaginal Examination द्वारा)


सामान्य देखभाल

  • रोगी को बिस्तर पर रखा जाए।

  • निरंतर निगरानी आवश्यक है और भ्रूण की हृदयगति (FHR) हर 5 मिनट पर दर्ज की जाए।

  • योनि परीक्षण केवल प्रारंभ में किया जाए, ताकि द्वितीय चरण की पुष्टि और किसी भी आकस्मिक नाल (cord) के बाहर आने का पता लगाया जा सके।

  • सिर की स्थिति और Station की पुनः जाँच की जाती है और यह सुनिश्चित किया जाता है कि सिर धीरे-धीरे नीचे आ रहा है।


प्रसव के समय उचित स्थिति (Position)

  • प्रसव के समय महिला की स्थिति लैटरल, स्क्वाटिंग या आंशिक बैठने (45°) की हो सकती है।

  • Dorsal position with 15° left lateral tilt को अधिक पसंद किया जाता है क्योंकि इससे aortocaval compression नहीं होता और धक्का लगाने में सुविधा मिलती है।


स्वच्छता और तैयारी

  • प्रसवकर्ता (accoucheur) को अच्छे से हाथ धोकर स्टेराइल गाउन, मास्क और ग्लव्स पहनने चाहिए।

  • रोगी की बाह्य जननांग और जांघों के अंदरूनी हिस्से को एंटीसेप्टिक घोल (Savlon/Dettol) से साफ करना चाहिए।

  • रोगी के नितंबों के नीचे और पेट पर स्टेराइल चादर बिछाई जाती है।

  • स्टेरिलाइज्ड लेगिंग्स का उपयोग किया जाता है।

Aseptic Technique – 6 Cs:

  1. Clean hands (साफ हाथ)

  2. Clean mother (साफ माँ)

  3. Clean delivery surface (साफ प्रसव सतह)

  4. Clean cutter (साफ काटने का औजार)

  5. Clean cord tie (साफ नाल बाँधने का धागा/क्लैम्प)

  6. Clean cord stump (साफ नाल का अवशेष भाग)


प्रसव की तकनीक

  • मूत्राशय भरा हो तो कैथेटर लगाना चाहिए।

  • रोगी को संकुचन आने पर जोर लगाने (bearing down) के लिए प्रोत्साहित किया जाता है।

  • जब भ्रूण का सिर लगभग 5 सेमी तक दिखाई देने लगे तो सिर को धीरे-धीरे झुकाकर (flexion) आगे लाया जाता है।

  • यह कार्य बाएँ हाथ की तर्जनी और अंगूठे से ऑक्सिपुट को नीचे और पीछे दबाकर तथा दाएँ हाथ से पेरिनियम पर दबाव देकर किया जाता है।

  • बार-बार यह प्रक्रिया संकुचनों के दौरान दोहराई जाती है जब तक कि subocciput symphysis pubis के नीचे न आ जाए।


सिर का जन्म (Crowning)

  • जब भ्रूण का सिर अधिकतम फैलाव (biparietal diameter) से योनि द्वार पर बिना पीछे हटे दिखाई देता है, इसे “crowning of head” कहते हैं।

  • यदि पेरिनियम फटने का खतरा हो, विशेषकर प्रथम प्रसव में, तो episiotomy (काटना) लिग्नोकेन (1%, 10 ml) से लोकल एनेस्थीसिया देने के बाद किया जाता है।

  • सिर का जन्म धीरे-धीरे संकुचनों के बीच करवाना चाहिए।

  • इससे भ्रूण का माथा, नाक, मुँह और ठुड्डी क्रमशः बाहर आ जाते हैं।


सिर बाहर आने के तुरंत बाद

  • मुँह और गले में जमी श्लेष्मा और रक्त को स्टेराइल गॉज़ से साफ किया जाए या सक्शन मशीन से निकाला जाए।

  • आँखों की सफाई स्टेराइल सूखी रुई से भीतर से बाहर की ओर की जाए।

  • गर्दन को हाथ से टटोलकर देखा जाए कि नाल (cord) लिपटी हुई तो नहीं है।

    • यदि ढीली है तो सिर/कंधे के ऊपर से निकाल दें।

    • यदि कसी हुई है तो दो Kocher’s forceps लगाकर बीच से काट दें।


कंधे और धड़ का प्रसव

  • कंधों को निकालने में जल्दबाजी न करें।

  • अगले संकुचन पर पहले anterior shoulder symphysis pubis के नीचे से निकलेगा।

  • यदि कठिनाई हो तो सिर को धीरे-धीरे पीछे की ओर खींचकर anterior shoulder निकालें।

  • फिर सिर को ऊपर की ओर खींचकर posterior shoulder बाहर निकाला जाता है।

  • कंधे निकलने के बाद उँगलियाँ बगल के नीचे डालकर बच्चे के धड़ को धीरे-धीरे बाहर निकाला जाता है।


बच्चे की प्रारंभिक देखभाल

  • बच्चा तुरंत एक ट्रे में रखा जाता है जिस पर साफ सूखा कपड़ा बिछा हो और बच्चे का सिर थोड़ा नीचे (15°) की ओर हो।

  • ट्रे को गर्भाशय से नीचे रखना चाहिए ताकि गुरुत्वाकर्षण से अतिरिक्त रक्त प्लेसेंटा से शिशु में आ सके।

  • श्वसन मार्ग (air passage) की सफाई सक्शन से की जाती है।

  • जन्म के 1 मिनट बाद Apgar स्कोर दर्ज किया जाता है।

  • नाल (cord) को 2–3 मिनट बाद या धड़कन बंद होने तक क्लैम्प करना चाहिए।

    • इससे 80–100 ml अतिरिक्त रक्त शिशु तक पहुँचता है।

  • नाल को दो Kocher’s forceps से पकड़कर बीच से काटा जाता है।

  • दो क्लैम्प/लिगेचर 1 सेमी की दूरी पर लगाए जाते हैं, जिनमें से पहला नाभि से 2.5 सेमी दूर रखा जाता है।

  • कटे सिरे को स्टेराइल गॉज़ से ढक दिया जाता है।


अंतिम कदम

  • बच्चे में किसी भी जन्मजात विकृति की जाँच की जाती है।

  • उसे सूखे और गर्म कपड़े में लपेटा जाता है।

  • पहचान पट्टी (Identification tape) माँ और बच्चे दोनों की कलाई पर बाँधी जाती है।

प्रसव के प्रथम चरण का प्रबंधन (Management of First Stage of Labor)

प्रसव का प्रबंधन प्रसव की शुरुआत से पहले ही प्रारंभ हो जाता है। प्रसव की घटनाएँ स्त्री और उसके परिवार पर गहरा मनोवैज्ञानिक, भावनात्मक और सामाजिक प्रभाव डालती हैं। महिला तनाव, शारीरिक दर्द और खतरों के भय का अनुभव करती है। देखभाल करने वाले को उसके प्रति संवेदनशील, सम्मानजनक और सहानुभूतिपूर्ण होना चाहिए।

प्रसव की तैयारी

  • वल्वा के बाल साफ (शेव) किए जाते हैं।

  • वल्वा और पेरिनियम को पहले साबुन और पानी से तथा फिर एंटीसेप्टिक घोल से अच्छी तरह धोया जाता है।

  • महिला को स्नान कराकर साफ कपड़े पहनाए जाते हैं और उसे चलने-फिरने दिया जाता है।

  • पूरे प्रसव के दौरान निरंतर प्रोत्साहन और भावनात्मक सहयोग दिया जाता है।

  • एक सुसज्जित लेबर ट्रॉली तैयार रखी जाती है।

योनि परीक्षण (Vaginal Examination)

  • जाँच रोगी को डॉर्सल पोजीशन में लिटाकर की जाती है।

  • जाँच का उद्देश्य महिला को समझाया जाता है।

  • हाथ और अग्रबाहु को साबुन और बहते पानी से धोया जाता है और नाखूनों को ब्रश से साफ किया जाता है।

  • स्टेराइल ग्लव्स पहने जाते हैं।

  • वल्वा को आगे से पीछे की ओर एंटीसेप्टिक घोल से साफ किया जाता है।

  • दाहिने हाथ की मध्य और तर्जनी उँगली पर एंटीसेप्टिक क्रीम लगाकर बाएँ हाथ से लेबिया अलग कर योनि में डाली जाती हैं।

  • पूर्ण जाँच के बाद ही उँगलियाँ बाहर निकाली जाती हैं।

  • संक्रमण के खतरे से बचने के लिए योनि परीक्षण न्यूनतम रखा जाता है।

पार्टोग्राफ में दर्ज की जाने वाली बातें

  • गर्भाशय ग्रीवा का फैलाव (Cervical dilatation): सेंटीमीटर में मापा जाता है।

  • Alert line: 4 सेमी फैलाव से शुरू होकर 10 सेमी तक 1 सेमी/घंटा की दर से।

  • Action line: Alert line से 4 घंटे दाहिनी ओर समानांतर खींची जाती है।

  • Effacement: ग्रीवा की पतलापन की स्थिति।

  • झिल्ली की स्थिति: साबुत/फटी हुई और यदि फटी तो पानी का रंग (साफ, मिकोनियम मिश्रित, रक्त मिश्रित)।

  • प्रस्तुत भाग व उसकी स्थिति।

  • Head Station: इस्चियल स्पाइन के संबंध में भ्रूण के सिर की स्थिति।

अन्य प्रबंधन

  • यदि झिल्ली साबुत है तो महिला को चलने-फिरने की अनुमति होती है।

  • उसे डॉर्सल सुपाइन पोजीशन से बचना चाहिए (Aortocaval compression रोकने हेतु)।

  • यदि मलाशय भरा हुआ हो तो साबुन-पानी का एनीमा या ग्लिसरीन सपोजिटरी दिया जा सकता है।

  • सक्रिय प्रसव में भोजन न दिया जाए। प्रारंभिक अवस्था में पानी, बर्फ के टुकड़े या जूस दिया जा सकता है।

  • महिला को स्वयं मूत्र त्याग के लिए प्रोत्साहित किया जाए। यदि संभव न हो तो बेड पैन दिया जाए।

मातृ निगरानी

  • नाड़ी (Pulse): हर 30 मिनट पर जाँचकर पार्टोग्राफ में डॉट (.) से चिन्हित।

  • रक्तचाप (BP): हर 1 घंटे पर तीर (→) से दर्ज।

  • तापमान: हर 2 घंटे पर जाँच।

  • मूत्र: मात्रा, प्रोटीन और एसीटोन के लिए जाँच।

  • दी गई दवाएँ: जैसे ऑक्सीटोसिन आदि, पार्टोग्राफ में लिखी जाएँ।

पेट का परीक्षण (Abdominal palpation)

  • गर्भाशय संकुचन की आवृत्ति, तीव्रता और अवधि नोट की जाती है।

  • 10 मिनट में संकुचनों की संख्या और प्रत्येक की अवधि (सेकंड में) दर्ज की जाती है।

  • पेल्विक ग्रिप: भ्रूण के सिर के ध्रुव (सिंसिपुट और ऑक्सिपुट) का धीरे-धीरे गायब होना देखा जाता है।

भ्रूण की निगरानी

  • भ्रूण की हृदय दर (FHR), उसकी लय और तीव्रता हर 30 मिनट में नोट की जाती है।

  • द्वितीय चरण में या झिल्ली फटने के बाद हर 15 मिनट पर जाँच की जाती है।

  • यह जाँच संकुचन के तुरंत बाद 60 सेकंड तक की जाती है।

मानसिक सहयोग

  • पूरी अवधि में महिला को लगातार मनोवैज्ञानिक समर्थन दिया जाए।

MANAGEMENT OF THIRD STAGE OF LABOR-

Ø  Third stage is very crucial stage of labor. Previously normal first and second stage can become abnormal within a minute with serious complications. 

Ø  The main aims in the management of third stage are to ensure strict vigilance and to follow  guidelines strictly in practice so as to prevent the complications, and  the important one is postpartum hemorrhage.

STEPS OF MANAGEMENT:

Two methods of management of third stage of labor are currently in practice.

Ø  Expectant management

Ø  Active management (preferred)

Expectant management

Ø  In this management, the placental separation and its descent into the vagina are allowed to occur spontaneously. Minimal assistance may be given for the placental expulsion if it needed.

Ø  The patient should not be left alone, A watchful continue monitoring should be done.

Ø  The mother should be kept in dorsal recumbent position

Ø  Fundus should be palpated-

Ø  (a) to recognize the signs of separation of placenta,

Ø  (b) to note the state of uterine activity—contraction and relaxation and

Ø  (c) to detect, though rare, cupping of the fundus which is an early evidence of inversion of the uterus.

Ø  Placenta is separated within minutes following the birth of the baby. A watchful expectancy can be extended up to 15–20 minutes

Ø   Watch for signs of placenta separations such as- uterine contraction and signs pain is seen on women’s face, gush of blood which stops in 10-20 seconds and cord may lengthen .  

Ø  At appearance of signs of placental serpration the women is asked to bear down simultaneously with the hardening of the uterus. The raised intra-abdominal pressure is often adequate to expel the placenta. most women will push the placenta out as soon as they feel pressure, with little effort.

Ø  As soon as the placenta passes through the introitus, it is grasped by the hands and twisted round and round with gentle traction so that the membranes are stripped intact.

Ø  If the membranes threaten to tear, they are caught hold of by sponge-holding forceps and in similar twisting movements the rest of the membranes are delivered

Ø  If the spontaneous expulsion fails, the palmar surface of the fingers of the left hand is placed (above the symphysis pubis) approximately at the junction of upper and lower uterine segment. The body of the uterus is pushed upward and backward, toward the umbilicus while by the right hand steady tension

Active management (preferred)

Ø  The Main principle in active management is to excite powerful uterine contractions within 1 minute of delivery of the baby (WHO) by giving parenteral oxytocic. This facilitates not only early

Ø  separation of the placenta but also produces effective uterine contractions following its separation. 

Ø  The advantages of AMTSL are

Ø  (a) to minimize blood loss in third stage  approximately to one-fifth and

Ø  (b) to shorten the duration of third stage to half.

Ø   The only disadvantage is slight increased incidence of retained placenta (1–2%) and consequent increased incidence of manual removal.

Ø  In active management of third stage of labor Injection oxytocin 10 units IM (preferred) or methergine 0.2 mg IM is given within 1 minute of delivery of the baby (WHO).

Ø  The placenta is expected to be delivered soon following deliveryof the baby.

Ø  If the placenta is not delivered thereafter, it should be delivered forthwith by controlled cordtraction (Brandt-Andrews) technique after clamping the cord while the uterus still remains contracted.

Ø  If the first attempt fails, another attempt is made after 2–3 minutes failing which another attempt is made at 10 minutes.

Ø  If this still fails, manual removal is to be done.

Ø  it should be applied in proper time and followed by rapid delivery of the placenta. It may be an ideal procedure while conducting delivery in an equipped

Ø  Examination of the placenta membranes and cord: - The placenta is placed on a tray and is washed out in running tap water to remove the blood and clots. The maternal surface is first inspected for its completeness and anomalies.

Ø  The maternal surface is covered with grayish decidua

Ø  Check membranes of fetal surface

Ø  examine the maternal surface to ensure there are no gaps, a gap would indicate a retained piece.

Ø   Look for accessory lobes (succenturiate lobes), placental infarcts, hemorrhages, tumors, or nodules

Ø  Weigh the Placenta: The weight of the placenta is often recorded

Ø  The membranes—chorion and amnion are to be examined carefully for completeness and presence of abnormal vessels indicative of succenturiate lobe

Ø  Assess the Umbilical Cord

Ø  Vessels: Count the number of arteries and veins (typically two arteries and one vein). 

Ø  Insertion: Note where the cord inserts into the placenta. 

Ø  Knots and Thrombi: Look for true knots or other blockages in the cord. 

Ø  Constrictions: Check for any constricting bands or webs along the cord.

MANAGEMENT OF SECOND STAGE OF LABOR-

The transition from the first stage to the second stage is evidenced by the following features:

Ø  Increasing intensity of uterine contractions

Ø  Bearing-down efforts

Ø  Urge to push or defecate with descent of the presenting part

Ø  Complete dilatation of the cervix as evidenced on vaginal examination

Ø  The patient should be in bed.

Ø  Constant supervision is mandatory and the FHR is recorded at every 5 minutes.

Ø  Vaginal examination is done at the beginning of the second stage not only to confirm its onset but to detect any accidental cord prolapse.

Ø  The position and the station of the head are once more to be reviewed and the progressive descent of the head is ensured.

Ø  Provide proper Position: Positions of the woman during delivery may be lateral, squatting or partial sitting (45°). Dorsal position with 15° left lateral tilt is commonly favored as it avoids aortocaval compression and facilitates pushing effort.

Ø  Proper scrubing should be done and puts on sterile gown, mask and gloves and stands on the right side of the table.

Ø  Prparation of external genitalia and inner side of the thighs is done with cotton swabs soaked in Savlon or Dettol solution.

Ø   One sterile sheet is placed beneath the buttocks of the patient and one over the abdomen.

Ø  Sterilized leggings are to be used.

Ø  Essential aseptic procedures are

    remembered as 6 Cs:

    (a) Clean hands,

        (b) Clean mother,

        (c)   Clean delivery surfase,

        (d) Clean cutter ,

        (e) Clean cord tie and

        (f) Clean cord stump

Ø  catheterize the bladder, if it is full.

Ø  The patient is encouraged for the bearing-down efforts during uterine contractions. This facilitates descent of the head. When the scalp is visible for about 5 cm in diameter, flexion of the head is maintained during contractions. 

Ø  Flexion of head is maintained by pushing the occiput downward and backward by using thumb and index fingers of the left hand while pressing the perineum by the right palm with a sterile vulval pad.

Ø  The process is repeated during subsequent contractions until the subocciput is placed under the symphysis pubis. 

Ø  At this stage, the maximum diameter of the head (biparietal diameter) stretches the vulval outlet without any recession of the head even after the contraction is over, and it is called “crowning of the head 

Ø  The purpose of increasing the flexion of the head is to ensure that the small suboccipitofrontal diameter 10 cm (4") distends the vulval outlet instead of larger occipitofrontal diameter 11.5 cm

Ø  When the perineum is fully stretched and threatens to tear especially in primigravidae, episiotomy is done at this stage after prior infiltration with 10 mL of 1% lignocaine.

Ø  Slow delivery of the head in between the contractions is to be regulated. This is done when the suboccipitofrontal diameter emerges out. This is accomplished by pushing the chin with a sterile towel covered fingers of the right hand placed over the anococcygeal region while the left hand exerts pressure on the occiput

Ø  The forehead, nose, mouth and the chin are thus born successively over the stretched perineum by extension.

Ø  Immediately following delivery of the head, the mucus and blood in mouth and pharynx are to be wiped with sterile gauze piece on a little finger.

Ø  mechanical or electrical sucker may be used. This simple procedure prevents the serious consequence of mucus blocking the air passage.

Ø  The eyelids are then wiped with sterile dry cotton swabs using one for each eye starting from the medial to the lateral canthus to prevent eye infection.

Ø  The neck is then palpated to exclude the presence of any loop of cord round the neck.

Ø  If it is found and if loose enough, it should be slipped over the head or over the shoulders as the baby is being born. But if it is sufficiently tight enough, it is cut in between two pairs of Kocher’s forceps placed 1 inch apart.

Ø  Delivery of the shoulders: Not to be hasty in delivery of the shoulders. Wait for the uterine contractions to come and for the movements of restitution and external rotation of the head to occur

Ø  During the next contraction, the anterior shoulder is born behind the symphysis. If there is delay, the head is grasped by both hands and is gently drawn posteriorly until the anterior shoulder is released from under the pubis. By drawing the head in upward direction, the posterior shoulder is delivered out of the perineum

Ø  Delivery of the trunk: After the delivery of the shoulders, the fore finger of each hand are inserted under the axillae and the trunk is delivered gently by lateral flexion

Ø  Soon after the delivery of the baby, it should be placed on a tray covered with clean dry linen with the head slightly downward (15°).

Ø  The tray is placed between the legs of the mother and should be at a lower level than the uterus to facilitate entry of more blood from the placenta to the infant by gravity.

Ø  Air passage (oropharynx) should be cleared of mucus and liquor by gentle suction.

Ø  Apgar rating at 1 minute  is to be recorded.

Ø  Delay in clamping for 2–3 minutes or till cessation of the cord pulsation facilitates transfer of 80–100 mL blood from the compressed placenta to a baby when placed below the level of uterus.

Ø  The cord is clamped by two Kocher’s forceps, the near one is placed 5 cm away from the umbilicus and is cut in between.

Ø  Two separate cord ligatures or plastic cord clamps are applied 1 cm apart proximal one being placed 2.5 cm away from the navel.

Ø   The cut end is then covered with sterile gauze piece after making sure that there is no bleeding.

Ø  A quick check is made to detect any gross abnormality and the baby is wrapped with a dry warm towel. The identification tape is tied both on the wrist of the baby and the mother
